                     DO YOU KNOW?

HOW TO FILE A CLAIM FOR SERVICE CONNECTED ISSUES.

In filing an initial claim for service connection, the veteran is required to submit VA FORM 21-526, "Application for Compensation or Pension."

The Post Service Officer can assist the veteran in completing this form and compiling the additional supporting documentation that should be submitted to the Department Service Officer for review before formally filed with VA.

The following is a list of some of the important documents and information that will help in preparing a "formal claim.

"A copy of the DD-214, Certificate of Release or Discharge from Active Duty." (If you do not have a copy of your DD214, it would be a valuable document to have on hand. (I can help you obtain this form.)

Copies of any service medical records or other service records would be helpful.
